This form allows for notice to be given to a tenant who is in default of the payment of rent. It provides for the amount of the defaulted payments to be specified and for a time limit to be placed on payment before further action is taken. It provides notice to either pay the rent or to vacate the property by a certain date. If the defaulted rent is not paid or the property is not vacated by the tenant within the time period allowed, a Notice to Terminate Lease may be delivered to the tenant which demands that possession of the property be relinquished. A lawyer should be consulted for further action, which may involve a lawsuit to enforce the lease terms, a lawsuit for collection of the past due rent, or legal proceedings for eviction of the tenant. A copy of the original lease should be attached to this form.
